                            RESOLUTION NO.   19761    
                   
                   
                   A RESOLUTION AUTHORIZING THE ADMINISTRATOR 
                   OF THE DEPARTMENT OF GENERAL SERVICES TO 
                   ENTER INTO AN AGREEMENT WITH C. J. HEALTH 
                   RECORD CONSULTANT SERVICES, INC., RELATIVE 
                   TO CONSULTANT SERVICES ON THE CITY'S RECORD 
                   AND INFORMATION MANAGEMENT ACTIVITIES, FOR A 
                   CONSIDERATION OF FOUR THOUSAND, TWO HUNDRED 
                   FIFTY AND NO/100 DOLLARS ($4,250.00), SAID 
                   AMOUNT TO BE TAKEN FROM THE CONTINGENCY 
                   FUND.
                   
                                                     
                   
                   
       	    	 BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF 
       CHATTANOOGA, TENNESSEE, That the Administrator of the Department 
       of General Services be and is hereby authorized to enter into an 
       Agreement with C. J. Health Record Consultant Services, Inc., 
       relative to consultant services on the City's record and 
       information management activities, for a consideration of 
       $4,250.00, said amount to be taken from the Contingency Fund.
